Complete this form to submit your ICC Columbian Award Qualifier. Once you click the Send button, a copy of this form will be sent to your Council's Grand Knight or Program Director and the State Program Director.
ICC Columbian Award Qualifiers are due by End of Day, March 1.

All fields in this section are required for submission.
Please provide three (3) programs your Council participated in during the Fraternal Year in each of the following categories (Faith, Family, Community, & Life).
One (1) program must follow the Supreme Faith in Action Guidelines. This activity will be listed first under each category. Select the Faith in Action Program your Council participated in from the Drop Down List provided.
The other two (2) programs can be selected from any of the programs/activities your Council participates in. Type the name of each Program into the text boxes provided. Only list one (1) activity per text box.
