Complete this form for each program that you would like to have considered for an On-Going Project Award. Once you click the Send button, a copy of this form will be sent to your Council's Grand Knight or Program Director and the State Program Director.

Some programs may be placed in different categories for award consideration based on Director's discretion.

On-Going Project Award Applications are due by
End of Day, March 1.
